NOTES ON 1982 LONG-TERM CARE SURVEYS

North Valley Health Care, Inc. - Stevensville reported a 186-day period
which was extrapolated to 365 days for data.

Butte Convalescent Center - Reported a 211-day period which was extrap-
olated to 365 days for data.

Four Seasons Nursing Center - Columbus reported a 182-day period which was
extrapolated to 365 days for data.

Bozeman Convalescent Center - Reported 182-day period which was extrapolated
to 365 days for data.

Livingston Convalescent Center - Patient Origin data is for January 1, 1982
to June 30, 1982. Numbers are extrapolated to a year by multiplying by
total admissions/P total admissions.
